In Geo News program 'Jirga' hosted by Saleem Safi, Aviation Minster Ghulam Sarwar Khan informed that a technical problem identified in December 7, 2016, PIA ATR 42-500 (AP-BHO) Chitral to Islamabad flight PK661 crash has been sent to ATR for re-examination and remarks.

Final crash investigation report will be presented in the parliament before the end of year 2020, the minister said.

Its 4 years ago, does it take that long. Besides the problem was in the engines that were made by Pratt & Whitney so why send it to ATR for re-examination. Seems like a delaying tactics.

If you look at the previous reports of engine related issues faced by ATRs in pakistan they all had the problem of having metal shavings on some sensor in the engine,AAIB had several times asked P&W to come up with a fix asap but it didn’t for some reason, The sensitive engine coupled with jerry rigging behavior of PIAs maintenance has wreaked havoc on the ATR fleet.

On October 6, Peshawar High Court (PHC) gave federal government forty-five days to present final investigation report of December 7, 2016, PIA ATR 42-500 (AP-BHO) Chitral to Islamabad flight PK661 crash.

Deputy Attorney General Aamir Javed produced Pakistan Civil Aviation Authority (CAA) letter issued on September 4, 2020, seeking more time to produce final crash inquiry report. Same request has also been made to Sindh High Court (SHC), where a similar petition on the air crash is being heard.

The court was informed that more time is required to complete investigation and finalize the report.

Today, Sindh High Court (SHC) gave federal government further ten days to submit final investigation report of December 7, 2016, PIA ATR 42-500 (AP-BHO) Chitral to Islamabad flight PK661 crash.

Earlier, the court had sought submission of report on October 15 (today).

October 27 is the last deadline given by Sindh High Court to Aviation Division for submission of final investigation report of December 7, 2016, PIA ATR 42-500 (AP-BHO) Chitral to Islamabad flight PK661 crash.

In October 15 hearing of the case, a government official informed the court that the report had been compiled and sought time to present it in the court.

In case hearing today at Sindh High Court, Aviation Division in its reply informed the court that December 7, 2016, PIA ATR 42-500 (AP-BHO) Chitral to Islamabad flight PK661 crash investigation is complete.

Crash report remarks/comments have been received by email from investigation teams from France and Canada. Crash report remarks/comments from USA not received so far. When received, reports from all three countries i.e. France, Canada and USA will be presented in the court, said the Aviation Division.

Court ordered Aviation Division to give copies of reports already received to the case appellant.

Next hearing of the case will take place on November 19.

According to Aaj News channel:

A preliminary crash investigation report by Pakistan's Aircraft Accident Investigation Board (AAIB) presented in the Sindh High Court today blames PIA for airline's December 7, 2016, ATR 42-500 (AP-BHO) Chitral to Islamabad flight PK661 crash.

The report says that the cockpit crew faced extremely complex technical problem in the aircraft.

Today Sindh High Court was informed that:

PIA was responsible for airline's December 7, 2016, ATR 42-500 (AP-BHO) Chitral to Islamabad flight PK661 crash.

Pakistan's Aircraft Accident Investigation Board (AAIB) says that cockpit crew faced a very complicated technical problem after ATR engine breakdown.

The technical problem experienced by flight PK661 ATR had never happened before with this aircraft type. It became impossible for the cockpit crew to control the aircraft.

The technical fault in engine developed due to aircraft's poor maintenance performed by PIA.

What was that very "complicated technical problem" which had "never happened" before and made it "impossible for crew to control the acft"?

Crash Investigation report opinion/remarks received from:

France as ATR aircraft manufacturer.
Canada as Pratt & Whitney Canada PW127 engine manufacturer.

Crash Investigation report opinion/remarks to be received from:

USA as Hamilton Standard engine propeller manufacturer and related systems.

Crash Investigation report opinion/remarks from USA expected to be received by October 30.

With crash report remarks received from all these three countries, complete report will be presented in the court.

Aircraft Accident Investigation Board (AAIB) informed the court that ATR’s failure sequence was triggered because of fracture of turbine blade consequent to one procedure anomaly of PIA maintenance adding that had any one of these latent factors not existed and triggered by a blade failure, this complicated catastrophic sequence of failures subsequently leading to the crash could have been averted.

This paper (as per avherald) was presented in the court. However it simply states dislodging of one blade in PT1 on no 1 engine, which resulted in erratic. There is nothing which is a first in aviation history, and does not explain how failure of one engine can result into something which pilots could not control?
