There were recommendations and safety advisories issued by AAIB after the ABQ 202 crash and NL 142 crash landing regarding the pilots training, their behavior and how they made approaches can anyone confirm that they were implemented? Especially the NL 142 one’s or are we repeating the same mistakes?

Cockpit crew communicated with Approach Radar instead of Control Tower, according to Geo News.

This is actually something that happens at karachi, a similar instance is when planes get cleared to land by approach control before being handed over to tower. Such instances should never occur.
They don’t get landing clearance by Approach if they have to be handed over to tower. Approach says “Continue Approach, contact tower 118.3”.

Approach can only give landing clearance if the aircraft has to be tuned in with them till the time they vacate the runway.

I asked same from a friend who regularly flies a private jet to OPKC . According him in these days of COVID19, a number of approach and ATC positions are handled by same persons. Often they find same person on radio after a frequency change. Cant say how true is this and is it allowed by rules.

You're right approach doesnt give landing clearences, but i have spoken to the captain of an a320 and he has told me that at least on one occasion, he was cleared to land before being handed over to tower.
Although this could be an isolated incident.
This doesn't really help us in the PK8303 accident but it may show us that SOPs aren't strictly adhered to by ATC.

Oh that could be a possibility.
I have heard of one controller controlling on two different frequencies, however I don't know if it is allowed for one person to control two positions at the same time, such as approach/tower.

Can anyone confirm this?

A great amount of focus is on the Captain.

As a general statement, any responsibility, praise, criticism should be shared. The Pilots try to do their job in a manner that allows them both to return home to their families. That takes care of the passengers and the rest of the crew too.

Mistakes and errors are part of being a human. Sometimes one thing can set off a series of escalating events, a cascade effect. The ‘why’ is much more important than the ‘who’.

One thing I’ve not seen mentioned and would like to ask. From the pictures of the respected Captain, I saw a star on each epaulet. Does this denote his rank as a Training Captain?
